We ask all new patients to complete their intake paperwork before their first appointment. This allows your provider to review your history in depth — so your time together is spent on you, not filling out forms in the waiting room.
Your intake form covers your health history, current symptoms, lifestyle factors, and your goals for care. Everything is submitted securely through your patient portal.

Plan to arrive a few minutes early for your first visit. Our team will welcome you, answer any questions, and make sure you feel settled before you meet your provider. First appointments are approximately 60 minutes — we never rush.
Health History Conversation
Your provider will sit down with you for a thorough review of your intake form and a genuine conversation about your health — not just your current complaint, but the full picture of how you've been living and feeling. This conversation shapes everything that follows.
Examination
A comprehensive chiropractic and orthopedic examination helps your provider understand the structural and neurological factors affecting your health. This may include postural analysis, range of motion testing, spinal palpation, and neurological screening.
Report of Findings & Care Plan
Your provider will walk you through what they found and what it means — in plain language, not medical jargon. Together, you'll discuss a customized care plan tailored to your goals, your schedule, and your body's needs.
First Adjustment (if appropriate)
If it's clinically appropriate and you're comfortable, many patients receive their first chiropractic adjustment during this initial visit. Your provider will explain exactly what to expect and answer any questions you have.
Common Questions
What should I wear?
Comfortable, loose-fitting clothing is ideal. You may be asked to change into a gown for the examination, which is always optional.
Is chiropractic care safe?
Chiropractic care has an excellent safety record. Your provider will conduct a thorough examination to ensure any care is appropriate for your specific situation.
Do I need a referral?
No referral is needed. Doctors of Chiropractic are primary care providers and you can schedule directly.
Do you accept insurance?
Please contact our office to discuss your insurance. We work with many plans and also offer transparent self-pay rates.
